To complete the enrollment application, you will need . . .
- Physician's name & phone number
- Dentist's name & phone number
- Health insurance company, policy number number, phone number, policy holder name
- Name & phone number of 2 emergency contacts other than parents/guardians
- Student's Social Security number

After you enroll, your application will be processed. Once it is, you will receive an email which includes a link to set up a scheduling appointment. At this scheduling appointment, you and your child will be able to talk to a faculty member about the available classes and class options that are available at FVL.

ENROLLMENT FOR FAMILIES WHO HAVE CHILDREN ALREADY AT FVL
- Log into your Skyward Family Access account (also linked in the Parent Portal) to register your child (not compatible with mobile devices).
- After you log into Skyward Family Access, click the "New Student Online Enrollment" link in the left column.
