WHO - Any girl in 5K through grade 8

REGISTRATION: 5k-4th Grade
                             5th-8th Grade

WHEN - Saturday, September 12
    Grades 5K-4 - 8 AM-NOON — Showcase 11:45-12:00 PM
    Grades 5-8 - 1:30-5:45 PM — Showcase 5:45-6 PM

WHERE    
    Fox Valley Lutheran High School gym
    5300 N. Meade Street in Appleton

COST - $40 per participant
This includes . . .

• sidelines and floor cheers
• dance routines
• cheer motions
• basic jumps
• stunting and tumbling 
• a custom hair bow

• NOTES    
  A $5 late fee will be charged for registration received AFTER August 30.
  If you register after August 30, you are not guaranteed a bow.
  The camp fee is non-refundable after August 30.
  Complete one form per participant. Submit the form and pay $40 by August 30.
   
   
• DRESS 
  Please wear non-slippery shorts, a t-shirt, and athletic shoes.
  Long hair should be pulled back in a soft hair tie.
  No jewelry or gum, please.
  Please remember to bring a water bottle for your athlete.
